Legal Name Change

Requirements

To change your legal name with Tufts, you will need a Social Security card, passport, or court-issued document showing your new legal name.

Students

1. Submit a request through your school's registrar. If you are in multiple degree programs, you'll need to submit information to each registrar.
   • ASE form
   • Medical School information
   • Dental School information
   • School of Medicine Graduate Programs information
   • Biomedical Sciences School form
   • Other schools: contact your school registrar for more information
2. After your registrar has confirmed the change, submit a request to it@tufts.edu to update your email alias/address and any other Tufts services you may use (see below).
3. Update your information in the Tufts Alert emergency notification system.
4. Email Public Safety to request a new ID card with your updated legal name at no cost. You will have to wait until your name has been updated in the directory (directory.tufts.edu).
5. If you are also a Tufts employee, you will need to complete a legal name change with Human Resources (see instructions below for Faculty/Staff).
6. Medford students only: Contact studenthealthinsurance@tufts.edu in Health Services to have your name updated in Medicat.

Faculty/Staff

1. Submit the Legal Name Change Request form to TSS. Please note, you'll be prompted to include a photo of your new Social Security Card.
2. After TSS confirmed the change, submit a request to it@tufts.edu to update your email alias/address and any other Tufts services you may use (see below).
3. Update your information in the Tufts Alert emergency notification system.
4. Request a new Public Safety ID card. You will have to wait until your name has been updated in the directory (directory.tufts.edu).
